
The Nursing Home Diversion Program is designed to enable people living with disabilities to live independently and remain in their homes rather than moving to a more restrictive and expensive setting.
The program provides home modifications, such as widening a doorway to accommodate a wheelchair; installing grab bars to make the shower safer; or building a ramp to provide easier access in and out of the home.
